Any individual working or living within the geographical plan service area who does not have insurance coverage.
What is the SmileCare Advantage Annual Fee?
Individual...............................................$75
Individual + One...................................$90
Family....................................................$105
Orthodontic Benefits Only................. $45
Your plan becomes effective on the first day of the month following CDS receipt of your enrollment application with the corresponding payment, provided both are received before the 20th of the prior month. Your plan becomes effective immediately if paid at a SmileCare Dental office.
